Tranexamic acid (often abbreviated TXA) is a synthetic derivative of lysine, an amino acid, that works as an antifibrinolytic agent—meaning it helps prevent the breakdown of blood clots (Acta Clin Croat, 2023). In skincare, researchers have identified several mechanisms that make it potentially useful: #1: it suppresses melanogenesis, the process your skin uses to create pigment (Acta Clin Croat, 2023); #2: it has anti-inflammatory properties; and #3: it has anti-angiogenic effects, meaning it may reduce the formation of small blood vessels (Acta Clin Croat, 2023; Clin Cosmet Investig Dermatol, 2024). Melasma—a common pigmentation disorder affecting people with darker skin tones—is the condition with the most research support.

For oral tranexamic acid specifically, studies show efficacy even at low doses (500 mg daily) over relatively short periods of 8-12 weeks in people with Asian skin (Dermatol Surg, 2018). Oral TXA appears most effective for melasma that hasn’t responded to standard topical treatments like hydroquinone, and #4: research suggests it does not increase the risk of dangerous blood clots, though patients need screening for contraindications beforehand (Dermatol Surg, 2018). However, oral forms do come with drawbacks: studies report gastrointestinal upset and menstrual irregularities in many patients (J Cosmet Dermatol, 2023). Beyond melasma, tranexamic acid is being explored for post-inflammatory hyperpigmentation (discoloration left after inflammation heals), rosacea (facial redness), acne-related redness, and other conditions involving both pigmentation and blood vessel issues (Acta Clin Croat, 2023; Clin Cosmet Investig Dermatol, 2024).

Topical tranexamic acid is less effective when used alone compared to oral forms, but #5: topical versions are better tolerated than hydroquinone and can be combined with other active ingredients to improve results (J Cosmet Dermatol, 2023; J Drugs Dermatol, 2023). Intralesional injections (injected directly into affected skin) and microneedling combined with topical TXA represent middle-ground options between topical-only and oral approaches (J Cosmet Dermatol, 2023). It’s important to note that for many skin conditions beyond melasma, tranexamic acid remains an off-label use—meaning it’s prescribed outside its officially approved indication (Clin Cosmet Investig Dermatol, 2024).
