All-trans-retinoic acid is a yellow to light-orange crystalline powder. (NTP, 1992) All-trans-retinoic acid is a retinoic acid in which all four exocyclic double bonds have E- (trans-) geometry. It has a role as an antioxidant, an antineoplastic agent, a keratolytic drug, a signalling molecule, a retinoid X receptor agonist, an anti-inflammatory agent, a retinoic acid receptor agonist, an AP-1 antagonist and a human metabolite. It is a vitamin A and a retinoic acid. It is a conjugate acid of an all-trans-retinoate.

🔬 The Science

Tretinoin (also called all-trans-retinoic acid) is a first-generation retinoid—a class of vitamin A derivatives that influence how skin cells behave (J Clin Med, 2025). Here’s what the research shows:

#1: Acne and photoaging. Tretinoin has “robust data from randomized clinical trials and systematic reviews” supporting its effectiveness for acne vulgaris and photoaging, making it the “gold standard anti-aging agent” (J Clin Med, 2025; Am J Clin Dermatol, 2024). It reduces signs of aging by stimulating epidermal growth and differentiation while inhibiting collagenase, an enzyme that breaks down collagen (Am J Clin Dermatol, 2024). #2: Other skin conditions. Smaller, less robust studies suggest tretinoin may help with melasma (a pigmentation disorder), postinflammatory hyperpigmentation (dark marks left after inflammation), striae distensae (stretch marks), flat warts, alopecia areata and androgenetic alopecia (hair loss conditions), hypertrophic scars and keloids, and actinic keratosis (precancerous spots).

It may also serve as a pretreatment before chemical peels or laser procedures. However, “high-quality, adequately powered trials with standardized outcome measures are still needed to establish clinical guidelines” for these uses (J Clin Med, 2025). #3: Skin cancer prevention. A large randomized clinical trial found that tretinoin does not prevent keratinocyte carcinomas (a common type of skin cancer), so it should not be relied upon for cancer prevention (J Clin Med, 2025). #4: Side effects and safety. “Adverse events are typically mild, localized, and transient”—meaning they occur in one area and go away—and “available evidence does not support an association with systemic adverse effects” (J Clin Med, 2025).

The main limitation is tolerability: most studies show that alternatives to tretinoin are less irritating and better tolerated, though tretinoin remains the gold standard (Am J Clin Dermatol, 2024). #5: Pregnancy and breastfeeding. For breastfeeding, topical tretinoin is considered low-risk because it is poorly absorbed through the skin. To minimize infant exposure, do not apply it directly to the nipple or areola, and ensure the infant’s skin does not touch treated areas. Avoid direct contact between the treated areas and the baby (a peer-reviewed journal, 2006). Oral tretinoin is a different story and should be avoided during breastfeeding and for 1 week after the final dose (a peer-reviewed journal, 2006).
